Pathway Care provides a comprehensive programme of care for children and young people with a range of disabilities. Our disability care service has been developed to assist Local Authorities in meeting the governments "Aiming High for Disabled Children Agenda" in England and "We are on the Way" agenda in Wales. This service will offer children and young people with disabilities the opportunity to become part of a family equipped to respond to their individual care needs.
Consultation between Pathway Care and Local Authorities has identified that there is a growing need for respite/short term breaks as well as longer term specialist care, to bridge this need Pathway Care has developed a fully comprehensive service for children and young people with disabilities
to include;
- Short Breaks
- Long term placements
- Life skills to assist transition to independence
- Care provision tailored to meet individual needs of a child placed
Short Break Care, what is it ?
Family based short breaks, link disabled children with families and individuals who can provide them with short term care on a regular basis. Most children who avail of this service have a learning difficulty, though they may have a physical disability, complex healthcare needs or a sensory impairment. They provide a vital support service to families of disabled children. Usually the child comes to stay at the short term carer's home and takes part in everyday activities.
It is recognised that with short term breaks, the children or young person's parent are and will remain the main carers and as such will remain central to the child or young person's care planning, particularly with regard to the promotion of health and educational needs.
Short Break carers.
Short break carers look after children with disabilities on a regular basis for short periods of time. They are fully trained Pathway Care foster carers that specialise in the care of Children with disabilities.
Our Carer's individual experience and tailored training programmes ensure that the individual needs of each child or young person can be met.
Each of the carers working with a child with disabilities is fully supported by a social worker who has a specialism within the realm of disability child care. Children and young people with disabilities will exhibit a range of needs that Pathway Care's specialist disability Care service will seek to match to the skill set and expertise of its foster carers. In addition to this support our carers have an independent support group dedicated to children with disabilities.
